The City of Ottawa Public Health Department requires that parents or guardians of children attending a school in Ottawa provide that school with a complete record of their child’s immunization.

Any child who has lived outside Canada for longer than three months in the last five years, cannot be permitted to attend school until the Health Department has received these records and the parent / guardian can produce a completed “Certificate of Immunization for School Entry”.

School Entry Immunization Forms will be provided by the Admissions team during registration.
